• Certificate of coverage request form

This online form is used to request a certificate of coverage for employees in receipt of superannuation guarantee.

This form can be used to submit:

    • a new request (up to 20 employees can be added on each form)
    • a request for an extension (submit one employee per form)
    • an amendment to an existing request (edit one employee per form).

The certificate of coverage will be sent to the postal address supplied in the form.

You may notice that there are now more requests appearing under the View previous request tab. This is based on a recent migration of previously submitted requests. Every effort has been made to migrate as many requests as possible, however this was not possible in all instances.

Where you are lodging an amendment or extension to a previously submitted request we recommend that you first search for the employee name under the View previous request tab as per the current instructions
