    We've provided the following information to help you submit forms online in the Tax Agent and BAS Agent portals.

    Submitting online forms

    You can complete the forms below while you are online by:

    • selecting and completing the form you want to send to us
    • selecting Submit to send the form.

    You must complete and submit your form in one session – there is no option to save a draft form.

    You will receive a receipt number when you submit your form. Use this number for all enquiries about your form.

    Some Tax Agent Portal forms request a tax file number (TFN). However, you do not need to quote a TFN. For more information, see Your privacy.

    Viewing submitted forms

    You can view most online forms that you have submitted in your Mail – Sent items.

    Select View submitted forms to see a list of forms you have sent to us.

    The online forms you can access will depend on which portal you are using.

    Auditor/actuary contravention report (ACR)

    Auditors have been unable to use the Tax Agent Portal for lodgment of ACRs since January 2013. For information about alternative lodgment methods, see Completing the Auditor/actuary contravention report.

    Electronic lodgment service (ELS) password reset form

    Use this form to request an ELS password reset. We'll phone you with your new password.

    Lodgment deferral form

    The Agent assessed deferrals, ATO assessed deferrals and Additional time to lodge for clients with overdue returns forms can be accessed via Lodgment deferrals – for registered agents.

    Once you have completed your form, you can lodge it as an attachment to a portal Mail message.

    Portal feedback form

    Use this form to provide us with feedback about the portal.

    The form allows you to type up to 1,024 free-text characters.

    You can clear the text you have typed by selecting Clear.

    Do not use the Portal feedback form to make enquiries about your practice or specific clients. Send us a message using the Mail function for your enquiry. 

    Private ruling application and objections

    Use this form to submit a private ruling application, an objection or provide further information about an application or objection. 

    Using this online form is the third step in preparing and submitting a private ruling application or objection through the portal. Before you submit this, you must first complete an application outside the portal.  

    When you select Private ruling applications and objections online form, the 'Private rulings and objections' screen is displayed. This screen provides information about the steps you need to take and links to our website to help you prepare your private ruling application or objection.

    The Private ruling application and objections online form allows you to attach the completed application and relevant files from your computer and submit them via the portal.

    Further information 

    The Further information link from the Private rulings and objections screen allows you to submit further information for a previously lodged private ruling application or objection. The previous private ruling application or objection doesn’t need to have been submitted through the portal. Further information you include can be information that we have requested or that you have identified that you need to submit. 

    To submit further information, you can:

    • type information directly in the online form’s text entry field
    • copy and paste from your own document
    • attach additional files for your previously lodged private ruling application or objection. 

    Request for replacement cheque form

    Use this form to request replacement refund cheques for your clients. Requests must be made on a client-by-client basis. Make sure the recipient’s contact details are correct before submitting the form.

    Tax practitioner complaints online form

    Use this form to refer an issue to us that hasn't been resolved by our regular channels within service standards.

    If you are escalating an unresolved issue, provide concise details about:

    • what has occurred
    • the outcome you are seeking
    • the steps you have taken to resolve the issue.

    Certificate of coverage request form

    This form is used to request a certificate of coverage for employees in receipt of superannuation guarantee.

    This form can be used to submit:

    • a new request (up to 20 employees can be added on each form)
    • a request for an extension (submit one employee per form)
    • an amendment to an existing request (edit one employee per form).
