• Commonwealth procurement statement

    Only use this request form if you apply for Commonwealth Government contracts worth over $4 million (including GST) and require a statement of tax record.

    The screen allows you to:

    • request a Commonwealth procurement statement (Statement of tax record)
    • view an online confirmation
    • access the receipt, finalised statement and taxpayer report.

    Completing the request

    To complete the request:

    • Select Online forms in the left hand menu.
    • Select Commonwealth procurement statement from the list.
    • When the Statement of tax record screen opens, you have the option to either:
      • apply for a statement
      • view the receipt, statement of tax record and taxpayer report.

    Select Apply for a new statement.

    You have the option to Cancel, or submit the request.

    If you submit a request, you'll receive an on-screen message telling you that your request has been successfully submitted. The on-screen message provides you with an ATO receipt ID and the date and time of submission.

    You can print or save a PDF version of the request receipt by selecting the Print-friendly version.

    Select the help function on the form for assistance with content.
