• Certificate of coverage request form

    Use this form if you are an employer requesting a certificate of coverage for employees receiving super guarantee. 

    You can use this form to submit: 

    • a new request (up to 20 employees can be added on each form)
    • a request for an extension (submit one employee per form) 
    • an amendment to an existing request (edit one employee per form). 

    We will send the certificate of coverage to the postal address you provided in the form.

    You may notice more requests appearing under the View previous request tab as we migrated previously submitted requests. Every effort has been made to migrate as many requests as possible; however it has not been possible in all instances.

    If you are lodging an amendment or extension to a previously submitted request we recommend you first search for the employee name under the View previous request tab.

    Authorisation to submit the form

    Make sure you have the correct permissions set in Access Manager to access the service. To view and submit the online form you need the relevant permission (view, prepare, lodge, revise) for the Certificate of coverage request form as well as the mail permission to view and action own mail. Your Access Administrator can login to Access Manager to set the permissions. 

    Without the permission, you will not be able to access the form in the online forms menu. 

    To update Certificate of coverage request permissions in Access Manager: 

    • go to Access Manager   
    • select Access and permissions under the My business heading in the left hand menu 
    • select the name of the user to be updated 
    • in the Permissions table under the Business tab, go to ATO transactions 
    • select the relevant boxes against Certificate of coverage request to view, prepare, lodge or revise 
    • select Save

    To set the mail permission in Access Manger: 

    • in the Permissions table under the Business tab, go to Administration 
    • select the box to view and action own mail 
    • select Save

    Information required

    To complete the request you will need: 

    • employee details – TFN (if provided), name, date of birth, place of birth 
    • secondment details (country, start date, end date) 
    • secondment address. 

    Get started

    If you submit the form as a new request, you can enter the details of up to 20 employees. When you submit (or lodge) the form, you will receive an on-screen confirmation of a successful submission and an ATO reference number. You can choose to print or save the form. 

    To complete the request: 

    • select Online forms in the left hand menu 
    • select the Certificate of coverage request from the forms listed ­– this form will only display if your Access Administrator has given the Certificate of coverage request permissions in Access Manager. 

    When the form opens, you have the option to: 

    • create a new request 
    • view previous requests – request an extension or amend an existing request 
    • resume a report that has been saved in draft. 

    Create a new request

    • Select New request – the company name and ABN will be pre-populated 
    • Complete the required information 
    • Select Add employee to add additional employees (up to 20) on the form 
    • Select Save employee to save each employee’s details 
    • You have the option to save and exit the form, or lodge the form 
    • Select Submit when the form is complete and ready to lodge  

    Request an amendment

    • Select View previous request 
    • Select Submitted requests 
    • Select Edit beside the form that you want to amend 
    • Review the information, then select Next 
    • Select the application type – Amendment  
    • Complete the required information  
    •  Attach up to six documents with a total file size of up to 10MB – they must be in the following formats: .doc, .pdf, .gif, .jpg, .png 
    • Click in the declaration box 
    • Select Submit  

    Request an extension

    • Select View previous request 
    • Select Submitted requests  
    • Select Edit beside the form that you want to request an extension for 
    • When the form opens, confirm the content, then select Next 
    • Select the application type – Extension 
    • Complete the required information 
    • Select Attach to upload and attach up to six documents (with a total file size of up to 10mb) with information supporting the extension request 
    • Acceptable document formats are .doc, .pdf, .gif, .jpg, .png 
    • Select Remove file to delete an attachment 
    • Click in the declaration box 
    • Select Submit 

    The Save and exit function in the form allows you to save the form in draft and return later to view or amend the form. 

    When a form is saved in draft, the form will be available to resume within 14 calendar days. After this time, the saved information will be lost. 

    The form declaration is electronically signed using the method of authentication that you used to log in. 

    Add additional employees

    To add additional employees (up to 20) click on the Add employee + button beside the Employee details field. 

    When the employee details are complete, you have the option to save the employee details or delete the employee details. 

    Once you have added an employee, you will be returned back to the form view. 

    Access a saved form

    • Go to Online forms in the left hand portal menu 
    • Select Certificate of coverage request from the list of forms 
    • Select View previous requests 
    • Select the Saved requests tab 
    • The saved form is displayed in a table with a Saved status 
    • Select View to access (resume) the saved form 

    If you don’t resume a form within 14 calendar days of saving it, all saved information will be lost. After this time the form will not display in the table. 

    Help completing the form

    The online form contains help text about general information and specific fields in the form. 

    Select the Help function on the form for help with form content.
